Students with Asthma need:

Information for New Families
We want to be proactive and address any of your child’s health concerns before the first day of school.
If your child has any chronic medical condition such as asthma, insect or food allergy, diabetes, or if you have any other concerns, please contact us.
We may wish to coordinate a meeting with you and your student(s) teacher in August.
If your student has a diagnosed insect or food allergy, please review and submit the appropriate forms:
- The Seven Hills Insect and Food Allergy Guidelines
- Emergency Care Plan – must be completed by the parent(s) & physician, preferably an allergist
- Medication Consent Form – must be completed by the parent(s) & physician, preferably an allergist
